Neelima Azim was born as Kanwal Azeem to a Marxist journalist Anwar Azeem. She was a trained Kathak dancer.
She was wed to actor Pankaj Kapoor. Shahid Kapoor is their son. The couple separated when Shahid was three years old. She wed another actor Rajesh Kattar with whom she has a son Ishaan. After their marriage ended, she wed for the third time to a singer, Ustad Raza Ali Khan.

Neelima Azeem was seen opposite Deepak Tijori in Sadak (1991). Since then she has played small roles in Zamana Deewana (1995), Itihaas (1997) and Ishq Vishq (2003) as Shahid Kapoor's mother.
